Online Athletic Training Questionaire Registration Page

Prior to participating on a team for Worcester State University, student-athletes must provide the Athletics Department with your current address, emergency contact information, insurance card, medical alert and health history information.
To help expedite this process, Worcester State uses an online data entry system. To enter your information please visit http://www.swol123.net.
The first time you visit the website:
1. Click JOIN SPORTSWARE
2. Worcester State University's school ID number is WSU2014.
3. Enter your college e-mail address and click Get Password. If you do not have worcester.edu e-mail address, please visit the helpdesk at the Learning Resource Center.

You will recieve an e-mail when your request has been accepted by
the school.
There is a PDF on our website that contains step-by-step instructions (with visual screenshots) which will help walk you through the registration process. Worcester State University's school ID number is WSU2014.
Please complete all of the forms that are required on your dashboard once you have registered. Please sign forms electronically by clicking save and submit. Also make sure you take a photograph or scan your health insurance card so you can upload it onto the SportsWare database.
